Virtual Wellness Program

Overview

Our Virtual Wellness Program provides convenient, personalized medical care from the comfort of your home. Through secure telehealth visits with our licensed providers, patients receive individualized treatment plans, ongoing medical guidance, and prescription medications when appropriate. Medications are shipped directly to your home, and our team remains available to support you throughout your treatment with regular follow-ups, education, and unlimited communication between appointments.

 

Billing

An initial consultation fee of $208 will be charged to the credit card at the time of booking. This fee is non-refundable.

If you elect to enroll in a monthly program, the consultation fee will be applied toward your first month’s membership.

 

Enrollment & Terms

By enrolling in any monthly wellness or GLP-1 program, you acknowledge and agree to the following terms:

    •    The first month’s membership fee will be prorated based on the date of enrollment.

    •    Payment is collected upfront for the current prorated month as well as the full fee for the upcoming month.

    •    Ongoing membership fees are automatically billed in full on the 1st of each month to the credit card on file.

    •    Enrollment cost is based on the number of medications requested by you and prescribed by your provider.

• Tier 1: $350/month — Includes Semaglutide + Vitamin B

• Tier 2: $450/month — Includes Semaglutide + 1 medication + Vitamin B

• Tier 4: $650/month — Includes Semaglutide + 3 medications + Vitamin B

Tier Change Request

Requests to move to a lower membership tier require a minimum of 60 days’ written notice. Most medications are prescribed and dispensed in 60–90 day supplies; therefore, a 60-day notice period is required to allow for appropriate medication management and billing adjustments.

Cancelation Policy

A minimum of 60 days’ written notice is required to cancel the program and must be submitted via text message or email. A final follow-up appointment is required to exit the program, and a $100 fee will be charged to the credit card on file. This appointment is required to ensure appropriate medical oversight and patient safety.

Shipping Address Changes
Patients are responsible for ensuring their shipping address is accurate each month. Any request to change a shipping address must be submitted in writing via text message to our office and received prior to medication shipment. It is patients responsibility to ensure the request was received. Verbal requests are not accepted to avoid address error. Updates can not be made verbally during appointments, as our medical team is clinical-only and does not handle administrative requests.

Lost, Damaged, or Discarded Medication Policy

Patients are financially responsible for all medications once dispensed. If medication is lost, broken, damaged, improperly stored, or accidentally discarded, it will not be replaced free of charge. The patient will be charged one full month of their current program fee to cover the cost of replacement medication. This policy applies regardless of cause, including accidental damage or disposal.

RxPortal

Access to your monthly, personalized treatment plan requires use of the RxPortal application. You will receive an invitation from our practice to download the app via text and email. When creating your RxPortal account you must use the email and number provided to our office. RxPortal is a HIPAA-compliant platform and is provided at no cost.
