Program Billing
An initial consultation fee of $208 will be charged to the credit card on file up to three (3) business days prior to your scheduled consultation. This fee is non-refundable.
If you elect to enroll in a monthly program, the consultation fee will be applied toward your first month’s membership.
Monthly Program Enrollment & Terms
By enrolling in any monthly wellness or GLP-1 program, you acknowledge and agree to the following terms:
• The first month’s membership fee will be prorated based on the date of enrollment.
• Ongoing membership fees are automatically billed in full on the 1st of each month to the credit card on file.
• Enrollment is based on the number of medications requested by you and prescribed by your provider.

• Tier 1: $300/month — Includes 1 medication
• Tier 2: $400/month — Includes 2 medications + B12
• Tier 3: $500/month — Includes 3 medications + B12
• Tier 4: $600/month — Includes 4 medications + B12
• Tier 5: $700/month — Includes 5 medications + B12
• Tier 6: $800/month — Includes 6 medications + B12
• Tier 7: $900/month — Includes 7 medications + B12
GLP-1 + Wellness Program — Monthly Membership Tiers
Semaglutide-Based Program
• Tier 1: $350/month — Semaglutide + B12
• Tier 2: $450/month — Semaglutide, B12, and one additional medication
• Tier 3: $550/month — Semaglutide, B12, and two additional medications
• Tier 4: $650/month — Semaglutide, B12, and three additional medications
• Tier 5: $750/month — Semaglutide, B12, and four additional medications
• Tier 6: $850/month — Semaglutide, B12, and five additional medications
• Tier 7: $950/month — Semaglutide, B12, and six additional medications
Tirzepatide-Based Program
• Tier 1: $550/month — Tirzepatide + B12
• Tier 2: $650/month — Tirzepatide, B12, and one additional medication
• Tier 3: $750/month — Tirzepatide, B12, and two additional medications
• Tier 4: $850/month — Tirzepatide, B12, and three additional medications
• Tier 5: $950/month — Tirzepatide, B12, and four additional medications
• Tier 6: $1,050/month — Tirzepatide, B12, and five additional medications
• Tier 7: $1,150/month — Tirzepatide, B12, and six additional medications
Peptides, Vitamins, Antioxidants
Medications available through the wellness programs may include NAD, MIC, Glutathione, Biotin, Vitamin B12, B-Complex, PT-141, and Sermorelin. All medications are prescribed based on clinical appropriateness and provider discretion. Medication offerings are subject to change in accordance with New York State regulations and clinical guidelines.
Tier Change Request
Requests to move to a lower membership tier require a minimum of 60 days’ written notice. Most medications are prescribed and dispensed in 60–90 day supplies; therefore, a 60-day notice period is required to allow for appropriate medication management and billing adjustments.
Cancelation Policy
A minimum of 60 days’ written notice is required to cancel the program and must be submitted via text message or email. A final follow-up appointment is required to exit the program, and a $100 fee will be charged to the credit card on file. This appointment is required to ensure appropriate medical oversight and patient safety.
Shipping Address Changes
Patients are responsible for ensuring their shipping address is accurate each month. Any request to change a shipping address must be submitted in writing via text message and received prior to medication shipment. It is patients responsibility to ensure the request was received. Verbal requests are not accepted to avoid address error.
RxPortal
Access to your monthly, personalized treatment plan requires use of the RxPortal application. You will receive an invitation from our practice to download the app via text and email. When creating your RxPortal account you must use the email and number provided to our office. RxPortal is a HIPAA-compliant platform and is provided at no cost.
