Program Instructions

Overview

Our Virtual Wellness Program provides convenient, personalized medical care from the comfort of your home. Through secure telehealth visits with our licensed providers, patients receive individualized treatment plans, ongoing medical guidance, and prescription medications when appropriate. Medications are shipped directly to your home, and our team remains available to support you throughout your treatment with regular follow-ups, education, and unlimited communication between appointments.

 

Video Call Appointment Instructions

  • These are video call appointments as required by telehealth guidelines, so please be prepared. Driving a vehicle during your appointment is not permitted.

  • There is no link required for this appointment. This is not a zoom. You will receive a video telephone call to your mobile phone.

  • Please be patient in the event our office is running 5-15 minutes late. We will attempt to call you three times. If you are unavailable we will likely have to reschedule your appointment.

  • Our medical team is calling from a HIPPA Privacy line which shows as a blocked number. Please enable blocked calls. If you miss their call please send a text message to our office at 516-619-2771 and we will notify them.

After your appointment

  • You will receive a call from our pharmacy within 1-2 business after your telehealth appointment to obtain medication payment and confirm your shipping address.

  • If you call the pharmacy prior to 2 business days from your appointment it is possible they have not yet processed your prescription. This does NOT mean your prescription was not sent. It means the pharmacy has not yet processed it.

  • After 2 business days please call the pharmacy directly if you have not heard from them. The pharmacy contact information is as follows: Enexia Pharmacy Telephone (718) 556-0942 Ext 8.

  • If you receive your medication and the cold packs have melted the medication is still to be used. The ice packs are an added precaution we take since FedEx does not provide temperature controlled transportation vehicles.

  • If you are not a candidate for your medication of choice, then you will be offered an alternative medical program.

  • If you decide NOT to proceed with the weight loss program, or you are NOT a candidate for your medication of choice, the Program fee payment of $208 is still required and non-refundable. Similarly, how you pay a co-pay at a doctor’s office when they accept insurance, this is our fee. This payment is not based on prescription receipt.

Booking Policy

  • Future monthly appointments are AUTOMATICALLY booked. If the assigned date and time does not work you please notify us immediately. Your confirmation text message will include date and time of your future appointment. Multiple attempts to cancel/reschedule will result in termination from our program.

  • Cancelation Policy: Cancelation or Rescheduling requests made within 14 days will incur a $75 rebooking fee.

  • Payment Policy: Credit Card is the only form of payment accepted. Your credit card payment will be processed up-to 72 hours prior to your appointment.​

Storage Instructions

1. Store your medication in the refrigerator. 

2. Keep away from light.

3. Do not freeze.

Shipping Address

1. Medications are shipped each month to the shipping address on file.

2. It is the patient’s responsibility to ensure the shipping address on file is accurate at all times.
3. Any request to change a shipping address must be submitted in writing via text message or email to our office. Verbal requests are not accepted to ensure no errors are made.

Injection Instructions

Semaglutide Pre-filled Syringes

1. Wash your hands thoroughly with soap and water.
2. Select an injection site: abdomen (at least 2 inches away from the belly button), outer thigh, or back of the upper arm. Rotate sites with each dose. Avoid areas that are bruised, red, tender, scarred, or irritated.
3. Clean the selected area with an alcohol swab and allow it to air dry completely.
4. Remove the needle cap from the prefilled syringe, taking care not to touch the needle.
5. Gently pinch a fold of skin at the injection site using your thumb and fingers.
6. Insert the needle straight into the pinched skin at a 90-degree angle.
7. Press the plunger slowly and steadily until all medication is injected.
8. Keep the needle in place for 5–10 seconds to ensure the full dose is delivered, then remove it straight out.
9. Release the skin fold and apply gentle pressure with gauze or a cotton ball if needed. Do not rub the area. Mild redness or tenderness is normal.
10. Dispose of the used syringe immediately in an approved sharps container. Do not reuse needles or syringes. If medication leaks, do not re-inject; contact your provider with any concerns.

Tirzepatide Pre-filled Syringes

What It Is

Semaglutide and Tirzepatide are prescription injectable medications used for medical weight management and metabolic support. These medications are administered as a once-weekly subcutaneous injection and are prescribed as part of a comprehensive medical treatment program.

How It Works

Semaglutide and Tirzepatide work by regulating appetite and hunger signals, slowing gastric emptying, increasing feelings of fullness, and improving insulin sensitivity and blood sugar regulation. Together, these effects help reduce caloric intake and support sustained weight loss when combined with proper nutrition and lifestyle habits.

 

Expected Benefits

Expected benefits may include reduced appetite and food cravings, improved portion control, gradual weight loss over time, and improvement in metabolic markers. Individual results may vary.

When to Use

These medications are administered once weekly on the same day each week as a subcutaneous injection. Common injection sites include the abdomen, thigh, or upper arm, and injection sites should be rotated regularly. Patients must follow the exact dosing and titration schedule provided by their medical provider and should not change their dose unless instructed.

 

Possible Side Effects

Common side effects may include nausea, vomiting, diarrhea, constipation, decreased appetite, bloating or indigestion, fatigue, and headache. These effects are typically mild to moderate and often improve as the body adjusts to the medication. Less common side effects may include dizziness, acid reflux or heartburn, and injection site irritation.

Contact Your Provider If You Experience

Patients should contact their provider promptly if they experience persistent or severe nausea or vomiting, inability to tolerate fluids, severe abdominal pain or pain radiating to the back, or symptoms suggestive of pancreatitis such as severe abdominal pain with vomiting. Patients should also notify their provider if they experience signs of gallbladder issues, including right upper abdominal pain, nausea, or fever. Symptoms of hypoglycemia, particularly in patients with diabetes or those using insulin, may include shakiness, sweating, dizziness, confusion, or a rapid heartbeat and should be reported immediately. Signs of an allergic reaction such as swelling or difficulty breathing require urgent medical attention so call 911.

Who Should Not Use

Semaglutide and Tirzepatide should not be used by individuals with a personal or family history of medullary thyroid carcinoma, Multiple Endocrine Neoplasia syndrome type 2, a history of pancreatitis, severe gastrointestinal disease such as gastroparesis, or known hypersensitivity to the medication or its components. These medications are also contraindicated in patients who are pregnant, trying to become pregnant, or breastfeeding.

Treatment Expectations

Weight loss with Semaglutide and Tirzepatide is gradual and varies between individuals. Side effects are most common during dose escalation periods. Ongoing follow-up appointments are required to monitor safety, tolerance, and response to treatment, and dosage adjustments may be made as clinically indicated. Optimal results are achieved when medication is combined with appropriate nutrition and lifestyle modifications.

Vitamin B12 (Methylcobalamin or Cyanocobalamin)
Some patients receive Vitamin B12 as part of their GLP-1 therapy to support energy production, red blood cell formation, and nervous system function. Vitamin B12 is generally well tolerated, and excess amounts are typically excreted by the body. In rare cases, administering higher than prescribed amounts may result in symptoms such as headache, nausea, diarrhea, anxiety, acne-like skin eruptions, flushing, new neurological symptoms, palpitations. Contact your provider if you experience any of these symptoms.

Patient Responsibility

Patients are responsible for following prescribed dosing instructions, attending required follow-up appointments, and communicating any side effects or concerns promptly. Patients must maintain adequate hydration and nutrition, store medication properly, and dispose of sharps safely. Patients acknowledge that results vary and are not guaranteed.
